Insurance Information


Eligible Plans

The Clinic is eligible for most health insurance plans.  Clinic contracted plans are listed below.  Insurance Co-Pays are expected at the time of service. 

High Deductibles or No Insurance

For patients and procedures where the patient's insurance will not be paying  for the visit, the patient must pay a minimum of $35 at the time of service.  The patient will be billed for the remainder.

We currently offer a 10% discount for full payment at the time of service.  We accept VISA and MasterCard.

 

 

Women's Healthcare

In Washington, women can now independently refer themselves to the provider of their choice for Women’s Health care. (As long as the chosen provider is associated with the woman’s insurance carrier)


  • Regence Blue Shield
    All PPO, Selections, private plans. 
  • Premera Blue Cross
    Most plans covered.  PPO plans.  All Women's health.   
  • First Choice Health Network PPO plans.  Most plans covered.
  • Uniform Health Care 
    Preferred Provider.  Primary Care Provider.  All women's health issues. 
  • Aetna:  Check with your plan.  PPO plans.  All women's health. .  
  • UnitedPPO, Women's Health.
  • TriCare  Temporarily open.  Check with your plan. 
  • Community Health Plan of Washington  Women’s Health. 
  • Molina:  Women's Health only. 
  • Medicare   Temporarily closed.  Recommend calling the Interfaith Clinic at 360-676-6177 for high quality health care.
  • DSHS  Practice is currently closed to DSHS.  Recommend calling Interfaith Clinic at 360-676-6177 for health care access.
  • Group Health:  Currently open to Group Health's Options Plan, but closed to Group Health's Standard Plan
  • Western Washington University’s Student Health Plan:  Will be open soon.  
  • Great West: PPO plans.
  • Labor and Industries:  Open
